The Health Buddy System has 360 day programs for long-term care coordination of patients with chronic conditions and 60 day programs for patients with chronic conditions who are post-discharge from the hospital. Analysis of data from a recent study in Medicare Fee-for-Service beneficiaries with the Health Buddy System, in conjunction with care coordination in medical group practices, demonstrated improvement in both clinical metrics and reduction in costs.

Bosch’s T400 Telehealth System supports multiple daily transmissions of vital sign and symptom data to care providers for patients who require careful monitoring of compliance with care post-hospitalization.
