Bosch Healthcare’s Health Management Programs (HMPs) have the unique distinction of being certified by the National Committee for Quality Assurance (NCQA). ). NCQA’s Disease Management certification standards are intended to help organizations achieve the highest level of performance possible, reduce patient risk, and create an environment of continuous improvement.
Bosch Healthcare’s 60, 180, and 360 sessions are developed using standard practice guidelines and focus on the monitoring of vital signs and symptoms, asking about healthy patient behaviors like medication adherence, and educating patients about their conditions in ways that promote better self-management. Our health management programs collect standard outcome measures, including healthcare resource utilization, patient satisfaction, quality of life, health management compliance and patient population reporting.
Our 60-day programs are designed to:
• Help prevent readmissions within the first 30 days by focusing on vital sign and associated symptom management
• Assist with the care transition process during the next 30 days by introducing content on health management behaviors and basic health education as the patient continues to stabilize and improve
